How to Submit a Referral
You can refer a patient to Detroit Oral Surgery by completing the online referral form above, or by calling our office directly at (313) 562-1515. We will confirm receipt and reach out to your patient within one business day to schedule their consultation.
What to Include
To help us prepare for your patient's visit, please include the following information with your referral: patient name, date of birth, contact phone number, reason for referral, any relevant radiographs or imaging, and your practice contact information so we can coordinate follow-up care.
What Your Patient Can Expect
Once we receive your referral, our scheduling team will contact the patient to book a consultation at our Metro Detroit office. During their first visit, our surgeons will perform a comprehensive evaluation, review imaging, discuss treatment options, and develop a personalized surgical plan. We keep referring doctors informed throughout the process and provide detailed post-operative reports so you can continue seamless care for your patient.
